Service Description:
Our community health visiting service is for families who have a child under the age of five. Health visitors provide a child and family focused service that promotes health and well-being and prevents ill health. All children under the age of five should have a named health visitor.

Referral procedure:

You can either refer yourself, be referred by your GP, or via social care, voluntary organisations or maternity services. Visit our website for more details.
